Speed up check-in by sending vaccination proof

For indoor services, we require an N95 or KN95 mask (we will have some available for a nominal fee if you don’t have one) and proof of up-to-date vaccination, meaning that you have received the full dose, plus booster(s) as you are eligible. 

To make it easier for you to check in to in-person, indoor events at UUCPA, we are accepting proof of vaccination via e-mail. Scan or take a photo of the front of your vaccination card, and send the image to directory@uucpa.org with “Vaccine” in the subject line, or put a photocopy in the mail to

Please do not send your original vaccination card! We cannot be responsible for these important documents.

The photos will be added to your private information in UUCPA’s Realm database, where they will not be visible to anyone except you and the database administrators. Each Sunday, John Wright and others at the welcome table will have a list of those who have given proof of vaccination, making your entry into the Main Hall quicker.

If you do not send proof in advance but attend on Sunday morning, John can snap a photo of your card when you present it in person for speedier admission the next week.
