Paula Wolfson, LCSW, Manager of Avenidas Care Partners, will review the varied recreational, health, wellness and counseling services offered at Avenidas. And then discuss strategies for developing mutual support systems to help elders socialize, thrive and also meet the challenges of lifestyle changes due to health issues.  Paula will review pro-active planning materials on health, legal, financial decision-making matters and ways to navigate these if you are a “solo” ager.

The session will include a Q and A period and will be facilitated more as a discussion group than a formal presentation.

Paula Wolfson, LCSW, manages Avenidas Care Partners program, an elder care consultation and case management service, well known for its vital services to elders in the South Bay. The ACP staff provide information and assistance-referral services, free geriatric case management for low-income seniors, elder care consults, educational workshops and private consults with older adults.
